What should you include? As much or as little as you want. As a medical professional, I would recommend the following:
Board Name, Full Name, Emergency Contact Name, Emergency Contact Number(s), Special Medical Conditions/Allergies, Current Meds. It may also be helpful to send me a description of the bikes you will have.
All information will be put into a document and a hard copy will be with me and a copy will be given to Ric. After the event, I do not keep your information (so don't say "same as last year").
Hopefully, we won't need this info, but you never know. Again please send to

I will not reply right away, so give me a week from the time you email me before asking if I got it. Once I open your email and get your info, I'll reply, otherwise I will leave the email "unread" so I know where I'm at in the process.
